El nervio supraescapular es el nervio motor para los músculos supraespinoso e infraespinoso. Esta compresión puede producirse tanto en la escotadura escapular, como en la escotadura espinoglenoidea. La escotadura supra-escapular es un lugar por donde el nervio supraescapular pasa por un túnel osteo-fibroso. La escotadura espinoglenoidea es la zona en la que el nervio discurre entre la espina de la escápula y la glena superior. En el caso de una compresión a nivel de la escotaduro espinoglenoidea, esta asociada frecuentemente a un quiste proveniente de la fisura del rodete glenoideo. Dicho quiste está formada de líquido sinovial que comunica con la articulación.
This nerve compression leads to dysfunction of the muscles that initiate superior and external movements of the shoulder, resulting in shoulder decentering. This is a rare condition that must be systematically ruled out in cases of posterior pain or significant muscle atrophy in the supraspinatus and infraspinatus fossae.
What risks were found?
The patient presents with a loss of strength with significant muscle atrophy, shoulder decentering, as well as posterior pain due to nerve compression.
What specific tests should be performed?
The exams to be taken are:
- an electromyogram (EMG) to test the suprascapular nerve
- an MRI to look for a cyst in the notch
- an arthroscan to detect a possible injury to the labrum and posterior rotator cuff
What treatment should be practiced?
In the case of compression of the suprascapular nerve, the treatment of this pathology is essentially surgical.
In the case of a cyst, the intervention consists of closing the opening through which the joint fluid leaks.
In cases of severe compression, this involves opening the ligament above the scapular notch. Recovery of function and pain is achieved in an average of three months.
